Adolbi Care's Community Reentry Homes represent the final phase in our comprehensive continuum of care for Children Without Placement (CWOP) and high-need adolescents and young adults. These homes provide supportive, semi-independent living arrangements for young people who have successfully progressed through our more structured programs and are ready for greater autonomy while still benefiting from ongoing support.
Our Community Reentry program is specifically designed to address the unique challenges faced by youth transitioning from institutional settings or foster care into independent community living. We recognize that this transition can be particularly challenging for young people who have experienced trauma, placement instability, or who have complex behavioral health needs.
Through our contracts with Superior Health Network and integration with Mental Health Rehabilitative Services (MHRS) and Mental Health Targeted Case Management (MHTCM), we provide a comprehensive support system that helps young people build the skills, confidence, and community connections needed for successful independent living.
Comprehensive support for successful community integration
Apartment-style living with minimal on-site supervision, providing a realistic environment for practicing independent living skills with a safety net.
Seamless access to Mental Health Rehabilitative Services including skills training, medication training, and psychosocial rehabilitation to support community functioning.
Dedicated case managers through our MHTCM services who coordinate care, connect residents with community resources, and provide ongoing support.
Assistance with job placement, career development, and workplace success strategies, including partnerships with local employers and vocational programs.
Support for GED completion, college enrollment, financial aid applications, and academic success strategies tailored to each resident's goals.
Access to individual therapy, group sessions, and psychiatric services as needed, with emphasis on applying skills in real-world settings.
Connection with peers who share similar experiences, facilitated community activities, and opportunities to build healthy social relationships.
Comprehensive planning for full independence, including housing search assistance, community resource connection, and ongoing support after program completion.
